Consent Information
Consent is central to the Focus Mindworks ADHD assessment pathway. Before you complete questionnaires, upload documents, book appointments, or begin treatment support, we want you to understand what information is collected, why it is needed, and who may see it.
Our assessment process may include screening information, clinical questionnaires, identity and contact details, supporting documents, payment information, appointment records, psychiatrist recommendations, and communication with your nominated GP or healthcare providers where clinically appropriate. We use this information to assess whether ADHD or another explanation may be present, to plan safe treatment, and to provide clear next steps.
You can ask questions before continuing. You can also choose not to proceed with parts of the pathway, although that may limit our ability to provide an assessment, medication recommendations, or shared-care communication.
What you are consenting to
By continuing through the assessment pathway, you consent to Focus Mindworks collecting and reviewing information needed to understand your symptoms, history, functional impact, health context, and treatment needs. This can include information you provide directly and, where you nominate them, information from trusted contacts or healthcare providers.
You also consent to the team using that information for clinical administration, appointment preparation, payment processing, documentation, and communication about your care.
GP and shared-care communication
ADHD assessment and treatment often works best when your GP or usual healthcare provider understands the recommendations. If a GP letter or shared-care communication is clinically appropriate, Focus Mindworks may prepare information that helps your provider understand the diagnosis, treatment plan, and follow-up requirements.
You can ask how information will be shared before it is sent. You can also contact the team if your provider details change or if you have concerns about shared-care communication.
Limits of the service
Focus Mindworks provides private ADHD assessment and related treatment support. It is not an emergency, crisis, or acute mental health service. If you or someone else is in immediate danger in New Zealand, call 111.
If you need urgent mental health support, use the crisis resources page rather than waiting for an assessment response.
